I, the client, agree to allow Hoggard Health Consulting Incorporated to collect and use my personal information and personal health information. For example, this could include name, telephone, email, date of birth, health history, medication list etc. This information is used to schedule, plan or deliver health services to the client either in person or using electronic communications.

Whereas the client is not the patient (substitute decision maker, health care organization), the client will take reasonable steps to:
-obtain consent of the patient and allow access to personal health information not directly available from the client required for the consultation
-obtain consent for communication to other health care team members outside the employment of Hoggard Home Health Inc.
-obtain consent for communication through electronic communications (email, videoconferencing, phone calls)
I, the client, agree to allow the consultant to share my personal health information and personal information with other members within my health care team. This may include, without limitation the client’s doctors, pharmacist, occupational therapist and dietician. The consultant will inform the client prior to of communications to other health care team members outside the employment of Hoggard Health Consulting Inc. so as to have an opportunity to withdraw consent.
The client acknowledges and accepts the risks of sharing personal health information through electronic (audiovisual or data) communications. The Client acknowledges and understands that while the Consultant takes commercially reasonable steps (encryption, passwords, etc.) to maintain the security of such personal health information, the Consultant cannot guarantee that such information will be secure. I understand that there are risks to electronic communication such as unauthorized access to my personal health information or technical failure during a consultation.
Each party (client and consultant) agrees that they will not disclose, divulge, reveal, report or use, for any purpose, any confidential information with respect to the other Party except as may be necessary in order to perform the Services or required by law. Each Party will ensure that the collection, use, disclosure, transmission, handling and storage of all personal health information is conducted in accordance with all applicable laws including without limitation the Personal Health Information Protection Act. The Consultant shall not use or disclose to any third party any patient information for any purpose other than as necessary to fulfill its obligations with respect to the delivery of the Services in accordance with this Agreement except with the consent of the Client or the patient or as permitted or required by applicable law.
